Important: COVID-19 Vaccination Record Submission


You are receiving this email because our records indicate that you have not submitted your COVID-19 vaccination record. You are required to be vaccinated for the Fall 2021 Semester unless you are approved for an exemption.  
The due date to submit your vaccination record was July 1. We understand that some students do not have access to vaccines and therefore have had to delay getting vaccinated until after July 1 or until they arrive on campus. Given the varying circumstances please complete one of the following steps by Wednesday, July 28It is critically important that we have the most up to date information from everyone about their vaccination status. 
  • Submit your vaccination record: You can submit your record by logging into the Student Health Services portal ( The portal can also be accessed from the Student Health Services website. If you have technical issues with your submission to the portal please contact Student Health Services via email (
  • Delayed vaccination but fully vaccinated before arrival on campus: Contact Student Health Services via email ( to inform them that you are in the process of being vaccinated with the intent to be fully vaccinated before arrival on campus. 
  • Vaccination upon arrival on campus: Contact Student Health Services via email ( if you need to be vaccinated when you arrive on campus. Please note that you will need to follow an onboarding quarantine process so please be in touch with your Class Dean as well so that they can coordinate your arrival with Residential Education and Living.   
  • Apply to be exempted from the COVID-19 vaccination: Contact Student Health Services via email ( requesting to be exempted from the COVID-19 vaccination requirement. Student Health Services will then follow-up with you on the steps to take to apply for the exemption.
