ACCIDENT/INCIDENT REPORTING
In the event of an accident, injury or near miss, supervisors and employees are required to fill out the Occupational Incident Report, and forward it to the Assistant Director of Employee Benefits in Human Resources.
H/R will send a copy of the completed form to the Director of Environmental Health & Safety for an accident investigation.
